Don't have allied victory on, and don't attempt to make any ceasefires or peace treaties...that could be your problem...
If you are absolutely sure that couldn't have been an issue, idk....did you save, then reload the game later? Since you can change difficulty after loading a game, the achievement may require the game to be done start to finish in one sitting, no save/loads....
