Are you by chance using an adapter to downgrade the DVI-out signal on your video card to the standard analogue VGA for the CRT monitor?
Have you tried using a different adapter? They are pretty cheap and a "bad one" could easily be the issue here.
I have tried with several adapter.
By the way, the anti-aliasing could be being "forced" by your driver settings (driver control panel). Please check to make sure it's set to "off" or "application controlled".
I have tried but it is always fuzzy.
Maybe that the only solution is to play with monitor lcd.
I will play always with my crt monitor, it's fuzzy but I have played 8 hours today but
I don't get migraines, it's ok.
