After installing the Stellar Phenomenon DLC I started noticing that Colonies would just up and die for no good reason, they weren't be attacked, no one was using superweapons on them and they were firmly in my culture. They'd just suddenly go dark. I've finally linked this to the more powerful pirate and/or rebel fleets that seem to randomly jump into sectors now.
